Social Security and Tax Identification Numbers

Your social security number is used by the university for reporting tax information on Form 1098-T. Although your social security number is not used as an identifier at the university and is not required for being admitted to UNO, you should ensure that it is on file with UNO to ensure you will receive a 1098-T form. This form is completed each year and is reported to the Internal Revenue Services (IRS) using a social security number.

An electronic version of your 1098T form for the prior year is available on Webstar under Student Self Service.

To apply for a social security number, please complete the Internal Revenue Service form http://www.socialsecurity.gov/online/ss-5.html. To apply for a taxpayer identification number, please complete form W-7 at http://www.irs.gov/pub/irs-pdf/fw7.pdf.
